The Annual Affiliation Form must be complete before every seasonal year by each organization. The Form is to be completed by July 1st.
This form ensures that all necessary information is up to date. The form must be completed by the club’s President and submitted once per season.
If there are any changes to the club’s information throughout the season, it is important to update the form and notify Eastern Pennsylvania Youth Soccer promptly.
Please Note: Clubs will not be able to start building their league rosters for the season if the Annual Affiliation form is not completed.
Complete the form in your organization’s GotSport Account.
1. Login at system.gotsport.com
2. Select ‘Affiliations’ from top menu bar
3. Select ‘Available Forms (2)’ and the Eastern Pennsylvania Youth Soccer Annual Affiliation Form for the appropriate year
4. Hit the ‘Select’ button to view and complete the form for the seasonal year
