Application Process:

Applications must be received no later than Friday, December 2, 2011. Metered mail is not acceptable for postmark date; only Postal Service stamp will qualify.

Please select your team’s competitive level request and your age group when registering online. Forward the completed application along with a copy of your league-approved team roster and a check for $300 payable to Eastern Pennsylvania Youth Soccer at: Indoor Cup Entry, Eastern Pennsylvania Youth Soccer, 4070 Butler Pike, Suite 100, Plymouth Meeting, PA 19462.

Refund Policy:

Refunds will not be provided after teams are accepted.
