The Certificates of Insurance that each Club has on file are automatically renewed each year and will be sent by email to the Insurance Coordinator as listed on the RG1 around the 20th of August.

 

Proof of Liability Insurance Request Form

In the event the owners or operators of game and practice facilities require proof of liability insurance before allowing member organizations to use those facilities, you can request a certificate from Eastern Pennsylvania Youth Soccer by completing the request form and returning it to Eastern Pennsylvania Youth Soccer, 4070 Butler Pike, Suite 100, Plymouth Meeting, PA 19462.

CERTIFICATE(S) WILL NOT BE ISSUED WITHOUT A WRITTEN REQUEST.
